Special Educational Needs & Disabilities

At Stoke Heath Primary, our intention for Special Educational Needs and/or Disabilities (SEND) is to ensure that all children receive a high-quality, aspirational and ambitious education regardless of need or disability. We believe that it is vital that our pupils are equipped with the tools needed to become independent, inquisitive learners both in and out of the classroom.

What is SEND?

A child has Special Educational Needs and Disabilities (SEND) if they have a learning difficulty or disability which requires special educational provision to be made for them.

Children may experience difficulties in one or more of the following areas:

  • Communication and Interaction

  • Cognition and Learning

  • Social, Emotional and Mental Health

  • Sensory and/or Physical Needs

How do we identify SEND?

Our teachers, SENCOs and senior leaders closely monitor the progress and wellbeing of all pupils using a range of assessments, observations and discussions with parents and carers.

Where concerns are identified, we follow the graduated approach outlined in the SEND Code of Practice:

Assess → Plan → Do → Review

This cycle helps us identify needs, provide appropriate support and evaluate its impact. Parents and carers are involved throughout the process and are informed if their child is added to the SEND register.

How do we support children with SEND?

High-quality teaching is the first step in responding to pupils who have SEND. Our teachers adapt lessons to ensure all children can access learning and make progress.

Support may include:

  • Adaptive teaching and scaffolding

  • Practical and visual learning approaches

  • Additional resources and equipment

  • Small group interventions

  • Individual support programmes

  • Individual Education Plans (IEPs)

  • Targeted support from trained teaching assistants

  • Recommendations from specialist professionals

Our aim is to remove barriers to learning and enable every child to succeed both academically and personally.

How do we measure progress?

Teachers regularly assess all pupils and carefully monitor the impact of any additional support provided.

Children receiving SEND support are reviewed regularly through:

  • Ongoing teacher assessment

  • Termly progress discussions

  • Parents' evenings

  • SEND review meetings

  • Review of intervention outcomes

  • Review of Individual Education Plans

Support is adapted whenever necessary to ensure it remains effective and meets the child's changing needs.

Working in Partnership with Parents

We believe that strong partnerships between home and school are essential.

Parents and carers are involved through:

  • Regular communication with staff

  • Parents' evenings

  • Termly SEND reviews

  • Shared targets and Individual Education Plans

  • Discussions about next steps and support at home

  • Workshops and curriculum events

We value parents' knowledge of their children and work collaboratively to ensure the best possible outcomes.

Pupil Voice

We encourage children to be actively involved in decisions about their learning and support wherever possible.

Pupils contribute through:

  • SEND review meetings

  • Discussions about their learning

  • Self-assessment opportunities

  • Pupil voice activities

  • School Council participation

Emotional Wellbeing and Mental Health

Supporting children's emotional wellbeing is a key part of our SEND provision.

Our support includes:

  • A comprehensive PSHE curriculum

  • Learning Mentor support

  • Time for Talk interventions

  • Boomerang programmes

  • Protective Behaviours programmes

  • Access to specialist services where appropriate

We promote a positive, nurturing environment where every child feels safe, respected and valued.

Specialist Support and External Agencies

Where additional expertise is required, we work closely with a range of external agencies and professionals to support pupils and families.

These may include:

  • Educational Psychology Service

  • Speech and Language Therapy

  • Occupational Therapy

  • Physiotherapy

  • School Nursing Service

  • Child and Adolescent Mental Health Services (CAMHS)

  • SEMHL Services

  • Early Years SEND Teams

  • SENDIASS

Accessibility and Inclusion

We are proud to be an inclusive school where diversity is celebrated. We strive to remove barriers to participation and learning for all pupils.

  • Lift access

  • Hygiene suite

  • Hoist facilities

  • Adapted learning environments

  • Specialist equipment where required

We work with specialist services to ensure that pupils can access both the school site and the full curriculum.

Transition Support

We recognise that transitions can be challenging for some children and carefully plan support to ensure children feel confident and prepared.

This includes:

  • Meetings between current and future teachers

  • Transition visits and activities

  • Sharing of SEND information

  • Additional support for vulnerable pupils

  • Close liaison with secondary schools
